Job Title/Position: Certified Registered Nurse Practitioner (CRNP)
Reports To: SBMC Executive Director/Collaborating Physician
CRNP Job Description Summary:
The CRNP (certified registered nurse practitioner) functions within a Collaborative Practice with the Medical Director/Collaborating Physician to provide medical onsite support for residents. This includes both review and monitoring of chronic conditions and issues as well as review, evaluation and interventions in response to acute conditions, for all residents in the skilled nursing facility/Living Assistance. The CRNP is responsible for providing medical care services, including assessment, diagnosis and clinical management of residents according to established standards of practice. The CRNP is an integral part of the clinical management team of the facility and works collaboratively with the DON, Unit Managers and other members of the interdisciplinary team to manage the clinical and medical needs of the residents. The CRNP communicates proactively with attending physicians regarding the clinical condition of their patients.
1. Current license as a Certified Nurse Practitioner in Pennsylvania.
2. Minimum of two (2) years nursing experience, geriatric and supervisory experience preferred.
3. Knowledge of community resources, documentation requirements and state regulations.
4. Demonstrated organizational and time management skills, excellent communication skills, interpersonal skills, documentation skills critical thinking, and ability to work independently.
5. Graduate of an accredited school of professional nursing.
Essential Job Functions/Responsibilities
o Collaborate with nursing staff and management to instruct and promote assessment skills of nursing staff
o Provide services and collaborate with the attending physician, facility care managers, nursing staff and Medical Director/ Collaborating Physician Chronic Care Management for patients with two or more chronic conditions expected to last 12 months, or until the death of the patient.
o Collaborate with the attending physician, facility care managers, nursing staff and Medical Director/ Collaborating Physician to coordinate services necessary to support resident care and treatment.
o Communicate daily with DON and Unit Managers to identify residents with clinical or medical urgent needs.
o Communicate with attending physician regarding findings and treatment plans ordered.
o Develop and order treatment plans.
o Discharge planning where appropriate. Engages with geriatric care management to facilitate discharge as needed.
o Document findings in resident’s medical record.
o Evaluate and manage acute problems and changes in condition of residents.
o Evaluate effectiveness of the treatment plan and make recommendations for changes when indicated.
o Initiate referrals and consultations with other licensed professional health care providers, and consult with other licensed professional health care providers as indicated.
o Interact with the resident and family/significant other providing information and support to develop and implement treatment plan as indicated.
o Interpret diagnostic tests.
o Order appropriate laboratory and diagnostic procedures.
o Participate in facility Quality Improvement process through meeting attendance, audit of outcome measures and input regarding processes and procedures.
o Participate in the facility team and/or family care conferences.
o Supports creation of care plans that reflect the values and goals of patients and families. Supports Advance Care Planning as indicated. Ensures adherence of clinical care to established care plans through ongoing communication with residents and their families / significant others, and with attending physicians.
o Perform medical evaluation of residents in response to identified need/request from nursing, attending physician or other staff.
o Perform procedures as needed in collaboration with attending physician/Medical Director/ Collaborating Physician.
o Provide Resident and family education and train other health care professionals as indicated and requested
o Provide clinical recommendations to nursing staff to address the medical needs of the residents.
o Provide onsite medical management in collaboration with Medical Director/Collaborating Physician and Attending physicians.

Licenses, Certifications, and Clearances
o Current state RN license and CRNP certification are required
o National certification, eligible for licensure by Pennsylvania board of nursing, eligible for certification as a CRNP by Pennsylvania state board of nursing
o Current CPR certification is required
